Problem with Shipping Yesterday

We have been running SC Pro 6.1.1 since it came out. We have set shipping
to be charged for all orders under $100.

Yesterday, we received a number of orders under $100 - some of them were
charged shipping, but most were not.

I checked our shipping settings and all seems fine. How do I tackle this
problem?

What shipping method are you using? How far under $100 were the orders
that were not charged shipping? What other
attributes/coupons/discounts/surcharges did the order have?
