Order errors in Shopsite 6.2.1 - Help please!

I recently upgraded to 6.2.1 on Solaris-Sparc. We have been seeing
intermittent errors ever since then. Here is the trouble: The customer adds
some products to their cart and completes an order. The customer receives a
correct email, and the order notification email is also correct, BUT: when
welogging in to download the order, 'phantom' products have appeared in the
order, always as the very last item. For example, see this order here:

Order #3375, Date: Tue Jan 21 01:42:01 2003

Qty. Name SKU Each Total
1 ps2008 - HP Carb Intake Manifold ps2008 $13.50 $13.50
2 ps2014 - HP Carb Gasket ps2014 $2.25 $4.50
1 s15 - Cool Head G230RC (3 Hole) s15 $42.00 $42.00
Silver
Sub Total $18.00
Tax Total $0.00
Shipping: Ground $0.00
Total $18.00

The product with SKU # s15 was not ordered by the customer, and does not
calculate in the total price, but it is displayed as part of the order.
Help, anyone?

We have seen 1 more order with this issue. (Typically we see 30-40 orders a
day). Unfortunately, the order was deleted by a sales staff member before I
could get to it. If the problem manifests again, I will let you know
immediately
