Php script - Contact Form

Hi,

I am having a simple HTML form on my Contact Us page, that calls the php script... Where do I upload the php script?
I have tried publish files (Includes)... and I have referenced this php script in my form as http://mywebsite/publish/script.php
but it's not working.

Please help.
