Subproduct Chcekbox

I would like to add check boxed options to my product along with my drop down's in checkout screen.

They are just optional products (added in my product list and added as subproduct to my product. How do I link these to sub products. example of what I would like on my checkout window:

Product
(dropdown option 1) (dropdown option 2)

Subproduct 1 - Additional Customization (checkbox)

(when checkbox is selected, adds price of product to total when re-calculated)

Subproduct 2 - Additional Languages (checkbox) (dropdown or fill in)

(when checkbox is selected, adds price of product to total when re-calculated)

This is something that you can do with a custom template, or some of our built in templates have this feature, subproducts displayed with checkboxes. The following themes all have this functionality:
