Help needed with custom checkout fields

I need to put a form on my checkout screen to gather more information from my clients.

I went to commerce setup-order system-checkout and created the form in the 'custom checkout fields' area.

How can I extract the HTML code from the custom fields that I have made and put it into the 'HTML Before the finalize button text box.

Any help would be greatly appreciated.
