ShopSite is adding two purchase products into one

I'm using ShopSite 9.0 Pro. Every once in awhile when customers order two or more products the products end up merging together in the ShopSite database. From what I can tell there's no rhyme or reason to when this problem happens and from what I can tell it is not product specific. For example, support a customer purchases the following products:

Widget A
Widget Z

When I look at the order in the back office I see the following:

Qty Name SKU Each Total
1 Widget A X9283 $2.00 $2.00
1 Widget Z:Widget A X83948 $3.00 $3.00

In this case "Widget A" gets added somehow to "Widget Z". I have no idea where the colon is coming from either.

Any ideas?

Widget Z is a subproduct of Widget A (or the other way around). If you don't want the name to appear like that then go to Commerce Setup > Order System > Shopping cart and check the box "Display subproducts without parent name:"
