Newbie needs to add form on cart

I am setting up my shopsite storefront. I need to add a form that captures client's birth information, and sends it to me once the order is placed.
Name at birth
Date of Birth (mm/dd/yyyy)
Time of Birth (hh:mm)
Place of Birth city, county, state, country

I can code it, I just don't know where to place this html code within the shopsite system to make sure I am copied on the text. Ideally I'd like it on the check out page Any ideas? All help is appreciated.

Try Under Commerce Setup--Order System--Checkout--Custom Checkout Fields...in Shopsite Pro, of course.

You set up the form fields using these options, then you can paste the code to actually show the fields in any of the 5 "HTML Before...." fields below.

Thanks for the help. I added the form code to the product page, and it seems to be working fine.
