How would one add a contact us form to a shopsite site? Does anyone know of a good script or method to add a contact form?

I would suggest contacting your hosting provider. Typically contact us forms will email the merchant the customer's information using the mail server of the host. I have seen a few hosting providers who get this request enough that they have a form that you can simply copy and paste onto your website to add this functionality.
