The information on the Ordering Instructions Box (on the Shopping Cart page)
says:

If this box is checked, there will be a box on the shopping cart page for
customers to enter comments about the items they are ordering. You can ask
for specific information or give instructions by entering text into the
"Text at the top of the Shopping Cart Screen" field.

Because of the way the Shopping Cart page is setup there is considerable
distance between the "Text at the top of the Shopping Cart Screen" and the
Ordering Instructions Box.

Is there a way to label the box "Ordering Instructions" just above the box?

By default there is a label directly above this field that says 'Ordering
Instructions', if that is not there on your screen, check your text
settings in Preferences -> Store Text -> Shopping Cart and make sure you
have the proper text entered there.
