Customer Registration Problem

One of our customers recently experienced the following problem:

When trying to log in to place his 4th order with us, his username/password did not work.

He used the function to e-mail a link to allow him to reset the password, and received the following via e-mail:

"We don't have you as a registered customer. Please register by clicking on the link below"

We asked him to try again, and this time he received a link allowing him to change his password. After doing this, he logged in and found all account info was gone: Name, Addresses, Payment Methods and order history.

Anyone else experiencing these kind of problems?
