Store Policy Check box

We are adding a returns policy in our check screen that requires a customer
to check a box indicating they have read our policy. When we check out on a
Mac system, no problems. When I use two different PC systems (using IE 6.0
or AOL) I get an error stating that "field01 is not defined" and the
transaction can not be completed.
VeriSign is requiring that we use this policy field.
Can anyone point me in the right direction to solve this problem? Thanks!
