International Shipping

I have a situation that deals with shipping internationally and
domestically.
The formula desired is as follows:
In the US: Flat $4.95
In Canada:: 10% of total order, $6 min/$15 max
Anywhere else: 20% or total order, $6 min/no max

Does anyone have recommendations on how to accomplish this, or of a plug-in
that would help?
I appreciate any suggestions.
