2nd ordering option on product

Is there any way to have a second custom order option for a product? If so, how to I do it?

Just put an extra return between the options for the first and second one.

Hmm.. to be more specific the product is a tshirt and...
I have the custom ordering options with the drop down menu to choose size of shirt.
But I also need a text input field for consumers to customize the shirt with their own text.
How would I go about doing that?

Go to Products > Edit Product Info and under the Ordering Options section there is a checkbox which will create a customer text entry field for the product.
