Header not dispalying correctly in Checkout or shopping Cart

Hi,

I have been using the order anywhere buttons to add to my first online store.
Things are progessing nicely but i am now staring to run into a few problems.
The first problem i am trying to eliminate is that my header banner is repeating itself in all the shopsite generated pages - (shopping cart, checkout, confirmation and thank you).
I don't know why it's doing this as the code has been copied and pasted from dreamweaver and as you can see , its fine in the home page or other pages not using shopsite.
Please keep explanations simple as i am new to all this.
