Adding custom text fields to a form

New to the group.

I found a standard template in the system I like and want to build my site around it to save time. I am building a rubberstamp website where clients will tell me what they want on their rubber stamp.

I am trying to produce a page that can take in extra lines of text field 1, field 2, field 3 to put on a label. Like on this website: http://www.winegiftclub.com/personalize ... ction.html
I want to be able to present in the shopping cart the same way they do it.

Questions:

What is the easiest way to do this?
Do I really need to create all new pages or can I just cut and paste into a exisiting copy of a product template page?
