Rush Order Fee for Air Shipping

I am currently offering flat rate shipping as my base shipping method, and users can pick UPS Ground or various UPS Air options if they need a guaranteed delivery time.

I would like to charge a $5 rush order fee for any of the UPS air shipments, as we baby these orders and rush them to the top of our shipping list.

Is there a way to have a $5 Rush Fee / Handling charge that is added only when these delivery methods are selected?

Other than a custom script, the only way I can think of is to use the surcharge field/drop-down menu in the Pro level cart. However, this would require the shopper to manually select the option.
