Custom template error: breaks when typing in zip code

I'm trying to use a custom header for my Checkout screen and have been
having trouble. I added a simple table with some images and links. Now when
a customer goes to check out, as soon as they type in their zip code, the
page breaks and the buttons don't work anymore.

This happens when I add the header by going into Commerce Setup>Order
System>Shopping Cart and putting it there OR when I add a header include
using custom templates.

Is this a known issue? The code I'm using is very simple - just table code
with a couple of images and links - no JavaScript or anything else. I'd sure
appreciate any help anyone can offer. Thanks!

No known problem like this. Maybe a url to the store so we can see what
your page looks like. (Have you run the source of the page through a
html validator like http://www.htmlhelp.com/tools/validator/?)

I placed a test order and it went through just fine. Have you made
changes that may have fixed the problem? What browser are you using? I
used Mozilla 1.5
