Shopsite: How did you hear about us module?

Do anyone know if there's a 3rd party module out there (or the functionality
in Shopsite, but it appears that there isn't) for a "Where did you hear about
us?" form for the checkout pages?

In a ShopSite Pro store you can use the custom checkout fields to put
this kind of information on the Checkout screen. The information will
be included on the merchant copy of the email receipt and in the
backoffice view orders section as well as the downloaded order files.
You could also write an Order API script to tabulate the information to
some external database if you want. I am not aware of a 3rd part
application that would do this though.

I have turned the Custom Fields On, but they do not show up. I have turned the Wish list(Which is assigned to Field1) off. But the Wishlist text appears and the Custom fields do not. Have there been any improvements to creating a "How did you hear about us" since 2005?

Besides just turning the Custom Checkout Fields on and configuring their names and types, you must ALSO add the HTML for your custom form fields to one of the 5 fields allotted for this in the Custom Checkout Fields configuration (which one you choose depends on where you want your custom fields to show up). If you put the HTML for the Custom Checkout Fields in one for the HTML fields as described and have the Custom Checkout Fields feature turned on but the fields still do not show up on your checkout page, then it is likely a problem with your custom shopping cart template.
