Need to add custom questions to order form

I am using Shop Site Starter (may need to upgrade) - but I need to add some simple questions to our order form and am really stumped. For example, one question is:
Would you like to volunteer for our events - yes or no (I can create radio boxes or fill in the blank response area, but NOTHING is being fed back to me on the order form).

So how/where do I put custom questions in which the response will be fed back to me when I receive an order? Do I need to upgrade to next level to be able to do this?

You are right, there isn't an option for this in ShopSite Starter (there isn't an option for this in ShopSite Manager either, only ShopSite Pro allows you to add your own custom checkout fields to the checkout screen). However, you can rig up your own form by taking the comments box on the shopping cart page, hiding it with CSS, then adding your own HTML onto the shopping cart page (you may need to fudge this as well in order to get it within the form fields) to add the additional information in the field that should be the comments box. This is definitely not a pretty way of doing this from a coding point of view, but it is doable. With you custom field(s) they need to use the same input name as the comments box so that your value(s) get(s) passed through for the rest of the checkout process.
