Surcharge error

We saw something odd today (I think). Someone ordered with a
surcharge -- we're using it for gift wrapping. The Surcharge in the
checkout shows the text associated with the $3 charge for gift wrapping.
However, there's $0.00 in the amount column.

We've had this configured and unchanged for a long time, and I believe
this is the first time we've seen this happen.

Here is the error case:

Subtotal $15.95
Shipping: First-Class Mail International $10.40
Surcharge: Red gift wrapping $0.00
Tax $0.00
Grand Total $26.35

And here's the correct case - what we usually see:

Subtotal $18.00
Shipping: First-Class Mail International $10.40
Surcharge: Red gift wrapping $3.00
Tax $0.00
Grand Total $31.40

And we haven't changed it, but I'll include the content of the Shopsite
surcharge configuration:

No gift wrapping; +0
Red gift wrapping; p +3
Green gift wrapping; p +3
Blue gift wrapping; p +3

Any ideas?
